HOW TO BECOME A NEW PATIENT AT SHCC
INSURANCE INFORMATION
All of our providers are accepting new patients at this time. We accept all insurances with the exception of a few plans listed here: Aetna Better Health of Ohio, Anthem Bronze Pathway, Buckeye Health Plan- Ambetter and Allwell, Humana Gold Plus HMO, Paramount Advantage, Premier Health Plan-Premier Health One and United Healthcare Community Plan.
HOW DO I MAKE AN APPOINTMENT?
To become a patient at SHCC, you will need to complete a new patient packet. You can download one below or stop by our office to pick one up. After the packet is completed, you can either mail it to us or bring it to our office located in the Kissell Center Building. After we receive the packet, in most cases, you can expect a call within 2 business days to schedule your first appointment!
