Print all pages of the packet. Make sure to print in full size (8.5" × 11") so all fields are clearly visible and there is enough space to write your information legibly.
Fill Out the Forms
Complete all sections of the packet carefully and thoroughly. Include your personal information, medical history, current medications, insurance details, and emergency contacts. Please use blue or black ink.
Submit Your Packet
Return your completed packet by dropping it off at our office during business hours, mailing it to 30 Warder Street, Suite 100, Springfield, OH 45504, or emailing it to [email protected].

Provider Matching
Based on your health needs and preferences, we will match you with the most appropriate provider on our care team.
We Call You
Once your paperwork is reviewed and insurance is verified, our receptionist will call you to schedule your first appointment with your provider.
Same-Week Availability
We strive to offer same-week appointments for new patients whenever possible. Our team will work with your schedule to find a convenient time.
